There are over 6.5 million adults living with heart failure according to The American Heart Association. This number is predicted to hit 8 million by the year 2030. Heart failure is a chronic condition that occurs when the heart muscles are unable to successfully pump blood throughout a body. This can present a wide array of symptoms including but not limited to shortness of breath, fatigue, cough, and swelling. Heart failure can be caused by a heart attack, chronic hypertension, heart disease, diabetes, or lung conditions. Though there is no cure for heart failure, a personalized treatment plan can help improve a patient’s quality of life while elongating their time with friends and family.
Baptist Life Communities Heart2Home Cardiac Program offers patients comprehensive cardiac care which begins while they are residents in one of our skilled nursing locations, and continues when they return home. While in house, patients receive around the clock nursing care, dietary consulting, daily weigh-ins, individualized therapy, laboratory services, EKGs and much more. Throughout their stay the same team of physicians, nurses, nursing assistants, dietitians, physical therapists, occupational therapists, and speech therapists will remain on that patient’s case. Afterwards, the goal of Heart2Home is to make the transition home as easy, safe, and comfortable as possible. Once home, we begin with a full home evaluation and personal assessment by a registered nurse. Patients will continue to have access to 24/7 nursing care and a variety of other services. Our own Heart2Home Director, Mary Thackston monitors each and every patient in the program, as well as educates staff, patients and their families.
